Overview

DDS Assistant Program Manager (Pioneer House). Responsible for coordination and implementation of in-house programming, including assessments, goal planning, evaluation of daily progress, monthly resident reviews, monitoring recreational programs, seeking community resources, and maintaining trip logs for van use. The role requires independent judgment, strong advocacy, and the ability to meet day-to-day needs of individuals served while ensuring services align with each person’s plan. Travel may be required; strong writing skills and clear communication are essential. The organization emphasizes a holistic approach to care, connecting individuals with internal or external services as needed and assessing needs of the whole individual and family, including basic needs such as food, clothing, and shelter. Follow-up on referrals and participation in cross-program service reviews are expected. Other duties as assigned. Responsibilities

Organize and maintain up-to-date records. Participate in Interdisciplinary Team meetings and Staff meetings. With the Program Manager, create meeting agendas and plan standard items (committee reports, announcements, staff concerns). Collaborate with consultants, especially Psychologist, on programming. Follow through on recommendations for U.R. committee and attend U.R. meetings when delegated. Complete and maintain investigation certification related to incidents within the DDS system. Ensure life-safety safeguards are implemented and followed. Maintain and submit timely, accurate health and safety records and program quality reports. Submit, update, and close all incidents for the program; maintain compliance with OPWDD and Justice Center regarding incidents. Make records available for review for IPR and UR. Aware of and sensitive to cultural and socioeconomic characteristics of persons served. Prepare staff schedules for Program Specialists, including vacation, personal, and sick leave and coverage during emergencies. Ensure appropriate staffing patterns per facility policy. Conduct scheduled supervisor meetings to discuss programmatic and staffing issues. Evaluate staff performance through observations and counseling in coordination with the Program Manager. Ensure individuals receive skill development support in activities of daily living, domestic living, recreation, and socialization as needed. Assist Program Manager in developing and enforcing agency/program policies per OPWDD or other funding guidelines. Conduct rounds on all shifts to ensure safety, a comfortable environment, and quality care and programming. Perform medical assistant responsibilities: track appointments, communicate with CMS, nurses, doctors; manage medication inventory and refills; track medical appointments; complete medical referral forms and monthly reviews. Coordinate transportation to maximize use of services for day programming, recreation, medical appointments, and other needs; ensure transportation of individuals to/from programs and activities. Complete Staff Information Entry Form when escorting individuals to medical appointments. Ensure availability of daily needs (toiletries, clothing, personal effects). Prepare Day Program progress reports for ITT reviews as assigned. Distribute petty cash funds and reconcile receipts as required. Conduct monthly fire evacuation drills with all staff on all shifts. Participate in admission and discharge reviews and provide recommendations to the Program Manager. Develop monthly recreation schedules in consultation with the Program Manager. Implement and monitor in-house and community recreational activities, providing hands-on assistance as needed. Other duties as assigned; direct care as needed. Attendance standard: 1:00 PM – 9:00 PM, Sunday through Thursday, subject to Program Manager’s discretion. Qualifications

High school diploma with one year of experience in Human Services (Social Work, Psychology, Special Ed., etc.) or related field required; Associate’s Degree preferred. One year of administrative experience. Minimum two years’ experience working with individuals diagnosed with intellectual disability (ID). Respect and consideration for residents being served. AMAP, SCIP-R and First Aid/CPR certification. Valid driver’s license required; ability to drive a large vehicle (e.g., wheelchair-accessible van).
