Summary:
The Cardiac Thoracic Care Coordinator is responsible for managing and coordinating comprehensive care for patients undergoing cardiac and thoracic procedures. This role ensures continuity of care from pre-admission through post-discharge by assessing, planning, implementing, and evaluating individualized care plans. The coordinator serves as a key liaison among patients, families, physicians, and the multidisciplinary healthcare team to optimize outcomes, enhance patient satisfaction, and promote cost-effective, high-quality care.
Education:
Graduate of an accredited school of nursing (Associate Degree required; Bachelor’s Degree in Nursing preferred).
Experience:
Minimum of 3 years of clinical nursing experience required.
Prior experience in cardiac, thoracic, or critical care strongly preferred.
Licensure:
Current and active Registered Nurse (RN) license in the State of Florida required.
Certifications:
Basic Life Support (BLS) American Heart Association required.
Advanced Cardiac Life Support (ACLS) American Heart Association required.
Skills And Abilities:
Strong clinical assessment, critical thinking, and organizational skills.
Excellent communication and interpersonal abilities to collaborate effectively with patients and multidisciplinary teams.
Knowledge of cardiac and thoracic disease processes, surgical protocols, and patient education techniques.
Ability to coordinate care efficiently across inpatient and outpatient settings to ensure smooth transitions and positive patient outcomes.
