Provides overall coordination in the delivery of medical services and discharge planning for specified patient populations. Performs concurrent and retrospective review of patient medical records for purposes of utilization review, care coordination compliance with requirements of external review agencies including government and non-governmental payers and quality assurance. Promotes a cooperative and supportive relationship as liaison with patient, family, facility staff, physician; funding representatives and community agencies. Ensure continuity in the handoff of patient clinical information from the hospital to other involved healthcare entities. Qualifications

What qualifications you will need: Experience Required

2 - 5 years Case Management Experience within the last year Minimum of at least two years of RN experience Education Required

Associate’s Degree-Major: Nursing Graduate of a nursing program: associate degree or baccalaureate program. Licenses And Certifications

Required: Current license to practice professional nursing in the state of Texas or Compact Nursing License. If your compact license is from a state besides Texas, you will be required to obtain a TX Nursing License within 90 days after your start date.
