Job Responsibilities
Psychosocial Assessment
– Performs comprehensive assessments of complex social situations using advanced interviewing skills and intuitive knowledge to identify challenges, root causes, and appropriate interventions. Includes advanced therapeutic intervention for abuse, neglect, and safety risk factors; reports suspicions to legally mandated authorities; and completes documentation with accuracy and clarity.
Resource Management
– Leads the identification and prioritization of interventions, eliminates barriers, and facilitates problem‐solving and conflict resolution. Empowers patients and families to be active partners with the medical team. Partners with patients/families to obtain financial assistance, community resources, or specialized equipment, and provides real‑time accurate resource information. Advocates at the system level and educates patients/families on medical, financial, educational, and social resources.
Psychosocial Interventions
– Provides psychosocial services per the patient’s comprehensive care plan, offers supportive counseling to maximize emotional coping and adherence, and collaborates with referral sources and the health‑care team. Uses evidence‑based treatment options and expertise in self‑management support for high‑risk, complex patients/families. Motivates and empowers patients/families through anticipatory guidance, planning, and family‑centered care team meetings. Demonstrates a therapeutic approach focusing on micro and macro systems to prevent problems.
Interdisciplinary Collaboration
– Promotes system improvement and patient/family well‑being, develops protocols to better meet patient population needs, mediates with the medical team, and initiates liaison roles between patients/families and outside agencies. Facilitates communication and coordination among health‑care team members and incorporates patient and family input in decision making.
Collaboration
– Partners with patients, families, and the health‑care team during safe transitions of care to the next level. Enhances resource knowledge, helps families become effective consumers of the health‑care system, and advocates for patients at the system level. Educates health‑care professionals and students about psychosocial aspects of care and leads efforts to improve division and team effectiveness. Promotes a positive work environment.
Job Qualifications
Master’s degree in Social Work or Clinical Counseling
5+ years of related work experience
Ohio LISW or LPCC license, or ability to transfer a license to Ohio upon hire
May require clinical license, certification, or training
Primary Location Anderson Outpatient
