Summary
We are hiring a PRN Speech Therapist! The Speech Therapist in Home Health is responsible for assessing and evaluating patient care needs related to treating speech and language disorders, as well as providing functional training in communication, swallowing, and cognitive impairments. Based on this assessment, the therapist helps determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates the patient's progress. Responsibilities
Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members. Evaluates the patient's functional level, status of body systems as required for CMS documentation, and speech therapy needs using standardized and/or criterion-referenced tools. Consults with the physician to develop the therapy plan of care. Treats patients' speech production, fluency, language, cognition, voice, resonance, swallowing, and auditory habilitation/rehabilitation following evidence-based standards. Observes, records, and reports to the nurse supervisor and/or physician the patient's response to treatment and any changes in condition. Instructs the patient, family, caregiver, and other healthcare team members in therapy areas within the scope of speech-language pathology. Visits patients across the service area and nearby agencies as needed. Completes progress notes for each visit and submits them per agency policy. Conducts patient evaluations, develops the speech therapy plan of care within state guidelines, and reports outcomes, goals, and projected care frequency within 24 hours of evaluation. Participates in staff conferences and committees as requested. Provides in-service education as needed and prepares discharge summaries or condition reports at least every 60 days. Adheres to therapy standards of practice and maintains current clinical knowledge and licensure. Flexibility in scheduling is required, including participation in weekend rotations and holidays, to meet patient needs. Education and Experience
License Requirements Must be currently licensed in Speech Therapy in Florida. Current CPR certification. Valid driver's license, vehicle insurance, and access to a dependable vehicle. Company Overview
