Job Description Summary In partnership with the Department of Health (DHS) and the Office of Diversion and Re-entry (ODR) Team, the ODR Case Manager will provide intensive case management to clients recently released from jail who are moving from homelessness to permanent supportive housing. Clients may have experiences of complex trauma and barriers related to medical, mental health, and substance use. The Case Manager collaborates with clients to develop independent living skills and other life-skills vital to success in maintaining permanent housing. Clients remain in the program for as long as they require subsidized housing.
Essential Duties and Responsibilities
Work with a diverse caseload of approximately 15 justice-involved clients with barriers including homelessness, complex trauma, medical, mental health, and substance use.
Maintain weekly contact with all clients and document contact in County-approved database.
Deliver services consistent with the "Whatever It Takes" model, emphasizing customer choice, harm reduction, trauma-informed care, and psychosocial rehabilitation.
Link clients to medical, mental health, substance abuse, and vocational resources.
Assist clients in obtaining benefits to which they are entitled.
Participate in court hearings and submit court progress reports.
Ensure each client on caseload has an up-to-date, comprehensive biopsychosocial assessment.
Collaborate with clients to develop creative goal plans to improve well-being and housing stability; review and update quarterly and upon goal completion.
Develop and maintain complete, accurate client files with required documents and data in agency records and electronic databases.
Document all client contacts in PIP format (Purpose, Intervention, Plan) and in accordance with Housing Department productivity expectations.
Maintain confidentiality of client files per HIPAA and applicable guidelines.
Coordinate appointments, transportation, and follow-up services for primary health care, mental health care, recovery services, and other community resources.
Transport clients to various agencies and healthcare providers in the greater Los Angeles area to support community integration.
Develop trusting relationships with clients to promote independence and improvements in physical and mental health.
Maintain knowledge of community resources to provide wrap-around services.
Assist clients with activities of daily living, scheduling and attending appointments, budgeting, socialization, health maintenance, sense of community, and recovery progress.
Work cooperatively with staff, including participation in weekly meetings and trainings.
Qualifications
High School Diploma or GED or equivalent
Bachelor's degree preferred; or a minimum of two years of case management experience with individuals experiencing homelessness, preferably with mental illness and/or substance addictions; or equivalent combination of education and experience
Valid CA Driver's license, reliable car, auto insurance, and acceptable driving record
Able to transport clients in own vehicle
Able to pass a background check for jail clearance
Demonstrated knowledge of case management
Able to work in a high-tolerance harm reduction model with clients who have multiple barriers
Skill in non-violent crisis intervention
Computer literate; ability to use computerized database for client file management with basic skills in Word, Outlook, and Excel
Detail-oriented with strong time management, organizational, written, verbal, interpersonal, and computer skills
Able to obtain and maintain CPR/First Aid certification
Preferred Qualifications
Bachelor's degree preferred
Minimum 30 WPM typing speed preferred
Work Environment
Combination of field and office environment
Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
Occasionally walk or drive to different local sites
Will work in busy and loud environments
Exposed to elements such as cold, heat, dust, noise, and odor
May need to bend, stoop, twist, and sit throughout the day
