Position: Physical Therapist (Full-Time) Base pay range: $43.00/hr - $55.00/hr

Qualifications
Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
Current license and/or registration in the state (as applicable).
Responsibilities
Provide a comprehensive physical therapy evaluation based on MD’s orders.
Document findings on the standardized evaluation form, accurately and adhering to all guidelines.
Develop a comprehensive treatment plan including long‑term and short‑term goals, frequency, duration, and modalities.
Ensure MD orders are obtained for evaluations, treatments, re‑certifications and discharges.
Utilize modalities and modify treatment as indicated while adhering to precautions.
Document patient’s daily participation and progress on weekly progress notes accurately and timely.
Update treatment plans as indicated.
Document discharge summaries and in‑service forms as required.
Complete all required documentation forms for the facility.
Maintain MDS care plans as deemed necessary by the facility.
Attend facility and rehab meetings, committees, and interdisciplinary team sessions.
Serve as a rehab delegate to the interdisciplinary team.
Schedule patients and perform screenings as indicated.
Submit accurate and ethical billing for physical therapy services.
Cover other therapists’ caseloads during absences as necessary.
Supervise junior staff members or students and co‑sign notes as required.
Maintain professional relationships with co‑workers, facility staff, patients, and families.
Provide in‑service education regarding patient care or physical therapy services to staff, hospitals, and the community as needed.
Comply with all facility policies and procedures, patient confidentiality, and federal resident rights.
Report to work on time, adhere to scheduled hours, and project a professional image at all times.
Adhere to productivity requirements.
Comply with all facility HR policies regarding lunch, sick leave, and time‑off notifications.
Maintain a positive work atmosphere by fostering positive relations with customers, clients, co‑workers, and managers.
Perform any other duties assigned by the department supervisor, lead therapist, or regional manager.
