The incumbent is accountable for coordinating the various facets of clinical programs and serving as a liaison between the facility and community programs and placement settings. Interdisciplinary teams develop, implement, and continually assess individualized treatment plans. At the Albert J. Solnit Children’s Center South Campus, staff treat youth with dignity and respect in a relational, culturally sensitive environment. Writes clinical program objectives, policies, procedures and manuals. Conducts individual health assessments. Develops, implements and reviews individual health care plans. Participates in or chairs interdisciplinary teams. Interviews individuals; accompanies individuals on visits to treatment programs or placement settings. Ensures continuity of care and follow-up in community programs to place individuals in the least restrictive environment. Assists with financial aid applications through the Department of Social Services as needed. Maintains records of individual progress or failure after placement. Develops community health resources and provides ongoing consultation and monitoring of services. Acts as a resource to staff and the community; prepares reports and statistical summaries. May supervise direct care staff and/or conduct in-service education. Performs related duties as required. Qualifications

Knowledge, Skill and Ability Considerable knowledge of psychiatric nursing and clinical therapeutic models with emphasis on interdisciplinary teams. Knowledge of psychiatric illnesses and medications; understanding of treatment planning and insurance requirements for reimbursement; awareness of service structures in institutional and community settings; knowledge of aftercare facilities. Strong interpersonal, oral and written communication skills. Minimum Qualifications General Experience:

Three (3) years of professional nursing experience. Substitutions Allowed:

A Bachelor's degree in nursing may be substituted for one (1) year of the General Experience; a Master's degree in nursing may be substituted for one (1) additional year. Special Requirements:

Possess and retain a license as a Registered Nurse in Connecticut or hold a Multistate/Compact RN license. Incumbents may be required to travel. About the Organization
