Overview
Case Manager III – Heartland Bridges (part time) role at Heartland Family Service. Heartland Bridges houses two programs, Bridges Transitional Living and Bridges Crisis Care. The Case Manager III provides direct care and intensive case management services to Bridges clients, delivering trauma‑informed, client‑centered support to help clients work toward stability in the community. Compensation and Schedule
Compensation: between $19.74 and $23.54 per hour (wage determined by total years of relevant experience). Work Hours: 24 hours per week (Monday, Friday, and Saturday, 7:30 a.m. to 3:30 p.m.). Minimum Qualifications
Associate degree in a human services field (including but not limited to psychology, social work, mental health counseling, marriage and family therapy, or a related field); Bachelor’s degree preferred Two years of experience in behavioral or mental health services One year of experience working with adults with serious mental illness or substance use preferred Valid driver’s license and acceptable driving record Essential Duties And Responsibilities
Responds to crisis care referral calls, assesses individuals by gathering information, and provides accurate application verdicts while maintaining strict confidentiality Completes enrollment/intake on eligible Crisis Care clients Develops supportive relationships with clients and responds to client needs during shifts Documents client activity per established record‑keeping procedures Monitors client self‑administration of medication and follows procedures for recording Executes full medication count on a nightly basis Conducts nightly housing lead searches Psycho‑educates clients on life, social, and coping skills Provides problem solving and crisis intervention services to empower clients to develop self‑sufficiency Utilizes trauma‑informed care, person‑centered, and culturally competent principles in providing services Demonstrates knowledge of current community and Heartland Family Service programs and resources to meet client needs Completes monthly client record reviews Conducts client room searches per program protocol Helps ensure room readiness for new clients Orients clients to Bridges protocols and addresses non‑compliance to maintain a safe environment Attends all staff meetings as scheduled Responsible for maintenance and upkeep of the facility Works independently with little supervision and as part of a team Maintains a courteous and caring attitude toward clients, volunteers, and visitors Abides by program and agency procedures and policies Cooperates and collaborates with program area staff, volunteers, and other agency staff Develops skills through participation in internal and external training opportunities Creates, maintains, and shares a dynamic self‑care plan Essential functions of this job are performed on the company physical work site Performs other program‑related duties as assigned
