Shift Schedule: Monday-Friday 9AM-5PM Hours Per Week: 35 Duties and Responsibilities

Under the direction of a Masters level supervisor, provide crisis intervention, protective services and social casework on a voluntary or involuntary basis to a caseload of mentally and physically impaired, abused, and/or neglected older clients; perform duties in accordance with social work standards, agency policies, and funding requirements. Conduct preliminary screening of applicants for service at intake; screen for program eligibility; triage for emergency intervention; ascertain short-term needs; provide accurate information and referral services to clients, families, and collaterals. Provide in-home and in-office assessments of social and emotional needs of the older adult client and family; utilize prescribed standardized assessment instruments (e.g., PSA, EISEP-PATH); secure cooperation of social work and professional consultants as needed. Develop and implement written case plans to provide a full range of social services for older adult clients and families; provide individual and family counseling and guidance to resolve problems. Prepare and disseminate information to clients and collaterals regarding development, challenges to functioning, and coping methods; inform about health, welfare, education and recreation services available to the older client. Screens client eligibility and applications for appropriate benefits; arrange for direct provision of services (homemaker, home health aide, public assistance, Medicare, Medicaid, emergency cash relief, legal aid, protective services, vocational placement, medical and psychiatric services, housing, etc.); evaluate clients’ capacity to manage their own affairs and protect themselves from financial exploitation. Assist in securing needed financial management or guardianship arrangements; disburse monies through JASA relief funds or client financial management services to secure goods and services. Refer to and maintain cooperative relationships with other community agencies to meet client needs; conduct ongoing in-home visits to evaluate client functioning, monitor risk, assess service status, and update the case plan with supervisor guidance as needed. Work with extended hours and emergency schedules as required, including beeper/phone coverage during evenings, weekends, and holidays on a rotating basis; respond to crises as needed. In crisis situations, be prepared to report to work outside normal hours, including evenings, weekends, and holidays, to support clients and initiate emergency actions. Supervise community aides, interns, and volunteers; represent and interpret the agency at community conferences and meetings; participate in training and unit meetings; complete required case records, reports, and statistics within mandated timeframes using computers if required. Handle special assignments and duties as assigned. Qualifications

Graduate of an accredited college or university with a B.S.W. or other BA-level degree or equivalent; preferred in psychology or gerontology. Good writing and record-keeping skills; ability to read, write, speak, and understand English. Must be computer literate, including ability to learn and use program-specific software. Ability to travel to clients\' homes using mass transit and to negotiate steps and barriers in clients\' residences. Knowledge of Yiddish/Russian/Spanish/Patois may be helpful.
