Case Manager Social Worker LCSW - Full Time Days at Northwestern Medicine Lake Forest Hospital. This is a full-time salaried position located at Lake Forest Hospital with hours 8:00 am to 4:30 pm and a weekend rotation required. Responsibilities

Develops individualized treatment plans for patients, in collaboration with the patient, their family and the health care team. Coordinates and leads care conferences for patients with length of stay greater than 7 days. Documents unusual occurrences and patient relations issues; makes appropriate referrals to risk management, infection control and quality departments. Assesses patient and family needs for appropriate discharge services; coordinates transfer to appropriate level of care (skilled care, rehabilitation, custodial and home health care) for complex cases. Documents discharge planning on the interdisciplinary plan of care, progress notes and education records (where applicable). Maintains current knowledge of community resources to support a continuum of effective services. Delivers notices related to Medicare discharge rights and discharge appeals for social work cases. Performs medical record reviews to assess appropriateness of admissions and continued hospital stay; monitors avoidable days and suggests changes in services or processes. Ensures compliance with managed care contracts and length of stay targets while maintaining quality of care. Develops and educates staff to reduce length of stay and resource use while maintaining quality of care. Identifies appropriate cases requiring social work services; collaborates with physicians and other healthcare personnel on social and emotional factors affecting health. Advocates for patients’ rights to access resources and improve compliance and quality of life; initiates goals of care conversations as needed; provides crisis intervention and supports families through illness, hospitalizations, and end-of-life considerations. Provides emotional support and counseling; connects patients and families with community resources to support social and health adjustment. Prepares patient histories, service plans and reports with accurate documentation of social work activities. Participates in ethics discussions and serves as liaison for discharge planning duties for inpatient cases as applicable. Qualifications

Master’s degree in social work Licensed Clinical Social Worker (LCSW) in Illinois 3 to 5 years of hospital or related setting experience Critical thinking, decisive judgment, and ability to work with minimal supervision Benefits
