Overview

RN II- Direct Care at Texas Health and Human Services Commission (HHSC). Location: CARLSBAD, TX. This position provides direct care in a State Supported Living Center (SSLC) setting, serving individuals with intellectual and developmental disabilities who are medically fragile or have behavioral needs. The role works with an interdisciplinary team to ensure high-quality, person-centered care. Responsibilities

Perform routine nursing tasks and procedures in accordance with HHSC, SSLC, and state regulations (e.g., wound care, tracheostomy care, gastrostomy/jejunostomy care, breathing treatments). Administer medications, immunizations, and treatments; monitor patients for symptoms and adverse reactions. Document and report treatments and observations in the medical record; prepare required reports. Assist with services provided by LVNs and Certified Medication Aides; participate in QA/QI initiatives. Respond to emergency situations; ensure safety of individuals and staff; provide education and safety training as requested. Support the Nurse Manager with controlled drug counts, medication counts, emergency logs, refrigerator temperature logs, and glucose controls. Participate in infection control and quality improvement initiatives; support compliance with safety and infection control procedures. Involve in disaster response, continuity of operations (COOP) activities, and may include alternate shift patterns or locations. Knowledge, Skills And Abilities (KSAs)

Knowledge of nursing techniques, health care regulations, medical diagnoses, and community health principles. Knowledge of state and federal laws and SSLC rules and procedures. Ability to provide patient care, maintain records, interpret policies, and understand health care regulations. Strong communication skills and ability to interact with individuals with intellectual/developmental disabilities, families, and staff. Ability to work independently and coach LVNs, CMAs, and DSPs; make decisions for SSLC leadership. Certifications and Eligibility

Licensed to practice as a registered nurse in Texas (or ARC with Nurse Licensure Compact reciprocity). Qualifications

Work experience as a practicing registered nurse is required; at least six (6) months preferred in a health agency, hospital, or health-care facility. Bachelor's Degree preferred. Additional Information

Applicants must pass background checks, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, CMA, and LEIE). State requirements on fingerprinting, I-9 documentation, and random drug testing apply. Flexibility in work hours; may require weekends and holidays; overtime may be required.
