Licensed Practical Nurse (LPN) Highland Place Rehabilitation & Nursing Center

Sign‑on bonus: $1500 for full‑time. Base rate: $26 per hour, increasing with experience.

General Purpose Provide direct nursing care to residents and supervise day‑to‑day nursing activities performed by nursing assistants, ensuring compliance with federal, state, and local standards.

Essential Job Functions

Direct day‑to‑day functions of nursing assistants per regulations.

Ensure compliance with policies and procedures.

Plan shift services, programs and activities, and coordinate with resident services.

Provide written and oral reports of shift activities.

Assist in development and implementation of quality assurance program.

Participate in annual surveys and inspections.

Review and update residents’ discharge plans.

Admit, transfer, and discharge residents as required.

Complete incident reports.

Charting and Documentation

Complete required forms/charts upon admission, transfer, or discharge.

Transcribe physicians’ orders to resident charts.

Record diet orders and report discrepancies to nurse supervisor.

Sign and date all entries in residents’ medical records.

Medication Administration

Prepare and administer medication per physician orders.

Verify resident identity and medication accuracy.

Maintain medication supplies and report needs.

Maintain accurate narcotic records.

Notify supervisor of drug discrepancies.

Nursing Care Functions

Deliver optimum resident care and comfort.

Respond promptly to call lights.

Consult with resident’s physician as needed.

Track and confirm administration of prescribed treatments.

Monitor seriously ill residents.

Review care plans daily.

Educate and supervise CNAs on care plans.

Other Duties

Attend in‑service education programs.

Follow Standard Precautions, Exposure Control, Fire Drill, and evacuation procedures.

Maintain confidentiality of records.

Protect residents from neglect and abuse.

Minimum Qualifications

Licensed Practical Nurse with current state license.

Excellent technical, assessment, and documentation skills.

Good leadership and communication skills.

Organized, detail‑oriented, and patient.

Compassionate for elderly and disabled patients.

Compliance with Residents Rights and Facility Policies.

Ability to perform physical demands required.
