Overview Professional I, Behavioral Health Transitional Care Specialist at The Centers. Provides short-term case management and care coordination for individuals with mental health needs transitioning out of inpatient care. Integrates primary and behavioral health care and substance abuse services. Collaborates with team; uses engagement skills; connects clients to health promotion and community services; maintains therapeutic relationships; facilitates warm hand-offs to permanent treatment teams.

Job Qualifications

A Bachelor’s degree in Social Work, Psychology or a mental health related field required. If in a related field, two years of care coordination or case management experience.

A valid driver license in Ohio with access to insured vehicle for client transportation.

Prior experience with severe and persistent mental illness (SPMI) adults preferred.

Working knowledge of stages of growth and development and family dynamics.

Job Skills

Essential Duties & Responsibilities

Provide short-term community psychiatric support and care coordination, including health promotion, comprehensive transitional care, individual and family support, and referral, linkage and advocacy to community resources.

Provide high-intensity engagement and follow-up.

Conduct level of care assessments.

Address social determinants of health: housing, social supports, economic needs, education, vocational needs.

Ensure hospital follow-up care recommendations are completed, including community psychiatric prescribing and nursing and community mental health case management.

Develop individualized treatment plans; perform reviews and updates.

Communicate vital treatment information from external providers to The Centers treatment team.

Work with Nurses, Physicians, Pharmacists to achieve positive outcomes.

Assist clients in obtaining and maintaining comprehensive healthcare; attend medical appointments as needed and transport as needed.

Provide ongoing communication to client and support network for follow-up and coordinated treatment.

Perform administrative tasks: complete clinical records, forms, and progress notes within 24 hours; maintain daily tracking log; meet weekly billable hours; other duties as assigned.
