Overview
Join the Cleveland Clinic team as a Licensed Practical Nurse (LPN) in Homecare Hospice. You will provide patient-first healthcare, support patients and families, and work with a dedicated caregiver team. This is a PRN position with a minimum of 48 hours every 6 weeks, including 1 weekend, and requires completing 8 weeks of full-time orientation (Monday – Friday, 8a–5p). Responsibilities
Actively engages and educates potential customers to the benefits and appropriateness of care. Ensure the patient and family have adequate knowledge of hospice or homecare benefits, as appropriate. For hospice only, process incoming referrals. Contribute to patient assessment by collecting data for RN review. Observe patients and report unusual conditions to professional staff. Observe safety risks and implement safety measures within scope of practice. Administer medications within scope of practice and monitor effectiveness and adverse reactions. Provide direct patient care including dressing changes and related laboratory tests as part of the plan of care. Provide ongoing education and information to patients and family, considering cultural and other challenges. Maintain complete and up-to-date documentation regarding patient condition and care interventions. Coordinate services at discharge to ensure seamless transition to the residential setting. Adapt priorities daily based on patient needs, family requests, and available resources. Other duties as assigned. Education
Graduate of an accredited practical or vocational nursing program. Languages
English required Certifications
Current Ohio License as a Licensed Practical Nurse required, with M-IV certification preferred. Basic Life Support (BLS) through American Heart Association (AHA). Valid Ohio Driver’s license upon hire; policy 8-175 thereafter. Proof of automobile insurance coverage with $100,000/$300,000 coverage upon hire; policy 8-175 thereafter. Competencies
Knowledgeable regarding JCAHO and Medicare standards and regulations. Ability to work confidently and accurately in a fast-paced, time-constrained environment. Ability to build and maintain relationships with patients, physicians, and community agencies while providing excellent service and care. Work Experience
Minimum of one year clinical experience as a professional nurse in an acute setting. In Hospice, previous hospice experience preferred. In Homecare, previous infusion experience preferred. Physical Requirements
Full range of motion, manual and finger dexterity, and eye-hand coordination. Standing and walking for extended periods; corrected hearing and vision to normal range. Exposure to communicable disease or bodily fluids; medium to heavy physical demands depending on tasks. Must be ambulatory and able to drive to patients’ homes, climb stairs, and assist movement of patients. Must be able to read, write, and communicate with patients. Personal Protective Equipment
