This role provides after-hours nursing coverage for patients on assigned weekends, holidays, and weeknights to support patients and families with questions, concerns, or emergencies. Staff may make home visits to provide symptom relief and/or support at the time of a patient’s death. This position collaborates with the manager on call and the weekend on-call triage nurse. Schedule The schedule for this position is Thursday-Sunday 10p-8a. Plan, provide, supervise and document safe, competent, and compassionate hospice nursing care for patients in accordance with physician orders and established policies and procedures. Provide after-hours clinical and emotional support for patients and families in crisis. Assist the triage nurse, when needed, in determining and completing appropriate coverage of after-hours calls from patients and families. Maintain current knowledge of nursing practices. Provide, and document, end-of-life information and education to patients and their families in compliance with CMS Conditions of Participation (CoPs). Participate in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational offerings. Manage time, people and technology professionally. Monitor and manage the appropriate use of patients’ medical equipment, supplies and medications. Collaborate with Interdisciplinary Group (IDG) members to determine the patient’s level of care and coordinate contract services as needed. Actively participate in the orientation and training of new employees of all disciplines. Work collaboratively with IDG and other Transitions LifeCare departments to achieve patient care goals. Delegate tasks and supervise the activities of LPNs and Hospice Aides as needed. Perform other duties as assigned by supervisor. Expectations

Registered Nurse with current NC license; BSN preferred. Minimum 3 years of recent clinical experience. Hospice and/or home health or nursing home experience or equivalent preferred. CHPN Certification preferred. Knowledgeable about reimbursement sources, Medicare regulations, licensing laws, and accreditation standards. Competent computer skills. Current CPR certification. Must have and maintain a valid North Carolina Driver’s License, automobile insurance, and access to reliable transportation. Must be able to travel to, enter and function in the homes/facilities of patients/families in the service area to provide care. May be required to lift and/or carry items up to 50 pounds. Must have sensory abilities to complete physical assessments and communicate with patient/family care providers and physicians, and to complete reporting and documentation responsibilities. Must be available to work on or around holidays as scheduled and potentially on a rotating basis. Must attend at least 4 team meetings per year.
