Integrated Care Coordinator
Cornerstone Care, a federally qualified health center (FQHC) serving Southwestern Pennsylvania and Northern West Virginia, is seeking an Integrated Care Coordinator to join our team.

Qualifications
Bachelor’s degree in Psychology or Social Work.
Behavioral health and/or medical clinical experience.
Knowledge of behavioral health, basic medical knowledge, and outpatient clinic experience preferred but not required.
Duties and Responsibilities
Provide patient care and assist in detection of at‑risk patients requiring further mental health and substance abuse screening.
Perform advanced screening of primary care/psychiatry/counseling patients for various mental health and substance abuse disorders.
Provide screening for social determinants of health, substance abuse, connect patients with appropriate services, assess barriers to treatment, and support chronic disease management.
Conduct brief behavioral and cognitive interventions and motivational interviewing to develop behavioral strategies for symptom reduction.
Provide psychoeducation and medical education materials for patients and their support systems.
Triage patients with severe or high‑risk behavioral health or substance abuse issues and refer them to appropriate specialists.
Facilitate and oversee referrals to outpatient and inpatient treatment programs and support smooth transition into treatment.
Provide brief, limited follow‑up visits for selected patients and contact patients post‑hospitalization when appropriate.
Assist in preventing relapse or morbidity in conditions that may recur over time.
Assist patients with medication adherence and disease self‑management techniques.
Consult with primary care or behavioral health providers onsite and by phone to enhance understanding and support treatment planning.
Maintain a visible presence in the community health center and participate in clinical staff meetings and quality improvement activities.
Educate primary care team on mental health and substance abuse disorders using brief behavioral interventions.
Train medical assistants and primary care providers to handle crisis situations and suicidality.
Assist in training psychiatry and/or medical assistants on referrals and mental health laws and regulations.
Report quality improvement measures, capture program evaluation data, and participate in quality management programs.
