Overview
Physical Therapist - Full Time (PT-FT), Adamsville. Ahava Therapy Services provides physical, occupational, and speech therapy services. Responsibilities
Review available resident information and evaluate its impact on the assessment and treatment process.
Conduct and document thorough functional resident assessments using standardized procedures.
Develop and implement individualized treatment plans with achievable functional goals.
Comply with evaluation, treatment, and documentation guidelines; maintain timely and accurate department records.
Instruct assistants in treatment procedures and provide close supervision when staff work with residents.
Enter treatment and progress notes in the resident record following each session.
Revise treatment plans as needed throughout the treatment process.
Document the course of treatment in a clear discharge summary using the prescribed format.
Orient residents to program services and procedures appropriate to their needs.
Collaborate with nursing staff, residents, and families to maximize functional abilities.
Instruct residents and family/caregivers about exercise programs and therapeutic procedures to continue at home post-treatment.
Assist in the referral process for additional services after discharge; maintain knowledge of community resources.
Attend or provide information for rehabilitation team conferences regarding resident progress, problems, or needs; participate in department/facility meetings as required.
Access continuing education opportunities to maintain clinical competency; participate in quality improvement processes.
Adhere to facility policies, procedures, and safety programs; uphold regulatory and professional standards.
Understand reimbursement by various payers and allocate therapy resources efficiently toward resident goals and discharge outcomes.
Maintain knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines.
Independently perform duties and recognize when direct supervision is needed; assist others as appropriate.
Maintain the expected productivity level for the position at the facility/facilities.
Qualifications
Bachelor’s or Master’s degree in Physical Therapy.
Current license in the state of practice.
Must have BLS CPR certification at hire or obtained during orientation; CPR certification must remain current during employment.
