Provide patient-focused, compassionate care in home health, hospice or related settings as appropriate. Travel to patient locations within the East Portland Metro area to deliver care. Collaborate with the care team to support the patient’s health goals and comfort. Qualifications
Graduate of an accredited LPN/LVN school of nursing Within 30 days of hire: National Provider BLS - American Heart Association Upon hire: Oregon Licensed Practical Nurse Washington Licensed Practical Nurse (within 60 days of hire) Driving is a necessary part of this role. Caregivers must comply with all state laws and requirements for driving and provide proof of driver license and auto insurance upon request. Upon hire: Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire Upon hire: Criminal background check compliant with Providence St. Joseph Health - OR policies At least 1 year of nursing experience in long-term care, acute care, ambulatory care or home health and hospice care Preferred Qualifications
