Responsibilities
Maintain annual HHRG values at the National Standard as appropriate.
Utilize HHRG values to establish visit frequency and interdisciplinary utilization.
Meet National Benchmarks for annual outcomes.
Refer patients to community resources as appropriate.
Evaluate patient clinical needs; develop, implement, and revise care plans; document accordingly.
Participate in interdisciplinary case conferences; evaluate care plan effectiveness.
Supervise and instruct Home Health Aide care plans every 14 days with documentation.
Notify MD/other care providers of significant patient condition changes.
Complete admission and other paperwork timely.
Provide clear narrative documentation reflecting skilled care, teaching, and patient response.
Obtain and clarify valid MD orders for all care.
Plan and coordinate discharges; inform patient/MD; record in discharge summary; complete discharge paperwork.
Perform all other duties as assigned.
Minimum Qualifications
Successful passage of VNA pre-hire medication test.
Current RN license in Massachusetts and related credentials for the specialty.
Minimum one year of nursing experience or 6 months with a BSN.
CPR certification.
Home care and IV experience, computer skills, and a BSN preferred.
Position Purpose Performs comprehensive nursing assessments upon patient admission, considering physiological, psychological, environmental factors, and health history to ensure safe, quality care. Integrates data to identify problems and develop care plans aligned with case management objectives.
Schedule Details 37.5 hours/week, non-rotating days, rotating weekends and holidays. Area: Mid to Lower Cape. Sign-on bonus eligible.
