Responsibilities
Provide primary medical care services in home, facility, and telehealth settings
Conduct physical exams, wellness visits, and manage chronic and acute conditions
Order and interpret diagnostic tests and labs
Prescribe and manage medications and individualized care plans
Coordinate care with interdisciplinary teams and community partners
Promote preventive care, patient education, and continuity of care
Participate in after-hours or on-call rotation as needed
Qualifications
Active Nurse Practitioner license in Louisiana
DEA license required
Experience in home health, hospice, or facility-based care required
Strong communication, decision-making, and clinical skills
Comfortable working independently across diverse care environments
Preferred
Facility-based or post-acute care experience
Familiarity with telehealth technology and EMRs
