Responsibilities
Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols Administer and order point-of-care and lab-based testing as needed Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities

Flexibility - Visits are scheduled based on YOUR availability. Assessments are conducted on Sundays through Saturdays from 8 am to 7 pm, with options for day, evening, and weekend work based on your schedule. Competitive Pay - Earn income based on the number of assessments completed. Monthly earnings range shown as an example depending on availability and location. Benefits - Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits. Education and Support - Onboarding training, one-on-one feedback, and access to continuing education. Additional Licensure - Support for obtaining additional state licensure and credentialing in neighboring states as needed. Full-time (FT) Opportunities - PRN providers can transition to FT roles and may receive a FT sign-on bonus.
Qualifications
Master’s Degree or commensurate experience with NP licensure Current NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority as needed AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care Current BLS, ACLS or CPR certification 1 year of NP experience preferred; new grads encouraged to apply Valid state driver’s license and proof of adequate automobile insurance for the state of residence Strong computer skills and familiarity with electronic health/medical record software Excellent verbal and written communication skills
