Case Manager Full Time Days

Pay Range Base pay: $55.97/hr - $88.87/hr
Shift & Hours Days / 7:00am – 3:30pm (40 hours per week)
Responsibilities
Assess all patients based on severity of illness and intensity of service for acute care and admission to appropriate settings (ICU, Med/Surg, OB, Peds, Rehab).
Interview patients or significant others to identify post‑hospital care needs and arrange ancillary services (HHC, HME, Hospice, transport, etc.).
Coordinate patient transfers between facilities.
Assess patient for appropriate level of care using Severity of Illness/Intensity of Service criteria.
Refer appropriately to allied health personnel and community resources.
Provide clinical consultation on difficult cases or initiate multidisciplinary care consultations as needed.
Communicate openly with physicians and Physician Advisors to facilitate discharge plans and deliver high‑quality care.
Serve as a resource for physicians, hospital staff, and outside agencies regarding insurance criteria and government regulation.
Deliver letters of denial, re‑determination, and referrals to patients, MDs, and the billing office upon notification.
Assist nursing in communicating to the Health Department regarding TB screening for authorized discharges.
Assist patients during interview process (e.g., turning, lifting).
Maintain confidentiality of departmental and patient information.
Demonstrate knowledge of age‑specific care, safety measures, physiological values/readings, skin integrity, behavior, and motor skills, and communicate effectively.
Pull files for TARs requested by physicians for billing.
Appeal authorization errors relative to payer criteria and processes.
Minimum Education
Graduate of an accredited RN Program.
Preferred Education
Completion of a Case Management Course.
Minimum Work Experience and Qualifications
Minimum two years’ experience as an RN in an acute care setting.
Ability to communicate effectively verbally and in writing.
Must be able to work in a union environment.
Preferred Work Experience
Minimum two years as a Case Manager.
Required Licensure, Certification, Registration or Designation
AHA BLS Card.
Current Los Angeles County Fire Card required (within 30 days of employment).
