Overview
Permanent Supportive Housing Case Management Specialist role with Wayne Metropolitan Community Action Agency. The PSH Program provides permanent, stable housing to formerly homeless individuals in Wayne County using Housing First principles. The PSH Case Management Specialist maintains a caseload of up to 20 households and delivers person-centered, trauma-informed services to help clients obtain and remain in stable housing. This position requires passion for social justice, teamwork, and a commitment to supporting people as they transition out of homelessness. Essential Functions
Provide supportive services and develop trusting relationships with individuals served by Wayne Metro’s Permanent Supportive Housing Program. Engage PSH residents face-to-face in homes and in the community. Meet or exceed weekly contact requirements for each PSH resident. Collaborate with residents to develop goals and individualized service plans supporting housing stability, mental health, substance abuse treatment, health, education, income, employment, and other priorities. Provide housing navigation services to newly assigned residents, including assistance with identification documents, housing applications, and viewing housing units. Support residents to maintain housing stability through tailored services and connections to community resources (e.g., tenancy problem-solving, independent living skills, benefits, employment assistance, money management, medical and behavioral health services). Act as liaison with landlords and community stakeholders to promote resident and program success. Collaborate with service providers to ensure high-quality, effective services meeting residents\' needs. Maintain timely and accurate records, compile reports, and complete program documentation while protecting resident confidentiality. Maintain a positive attitude and professional conduct; uphold Wayne Metro policies and procedures. Other duties as assigned. Minimum Qualifications
Bachelor’s degree in Human Services, Public Administration, Social Work, Business Administration, or equivalent combination of education and experience. Minimum of four years of related experience. Knowledge of Detroit and Wayne County demographics and needs of low-income populations. Ability to design and implement outreach, education, and information/referral services for individuals facing barriers. Ability to collaborate with area social service providers to coordinate and modify services. Ability to collect and evaluate data, identify service gaps, and develop strategies to address them. Ability to supervise staff and coordinate activities across sites. Ability to design and implement new activities including program design and operation. Strong oral and written communication skills. Empathy and effective communication with individuals facing barriers. Valid driver’s license and reliable transportation. Exceptional interpersonal and professional skills including appearance, tact, punctuality, and dependability. Minimum Competencies
Proficiency in Microsoft Office (Word, Excel, PowerPoint) and Google tools. Ability to evaluate programs and propose efficiencies. Strong communication skills in both oral and written form. Compassionate, respectful service delivery to individuals facing barriers. Ability to work independently or in a team. Adherence to Wayne Metro policies and procedures. Travel
Ability to travel to various sites as needed. Licensing and Other Requirements
Valid identification and reliable transportation. Covid-19 vaccination is encouraged; Wayne Metro prioritizes staff and client safety. Work Hours
