: Targeted Case Manager Job Summary
: Emerald is seeking individuals with a passion for helping individuals identify the strengths within themselves to overcome obstacles and barriers. Targeted Case Management focuses on strengths and abilities to enhance skill sets, enhance social networks, and link to resources in the community that would be useful to clients. This may include access to housing, medical, social, educational, and other identified services. Clients are diagnosed with SUD (Substance Abuse Disorder), SMI (Severe Mental Illness), and SED (Severe Emotional Disability). This position is responsible for managing a caseload of up to 25 clients, including face-to-face time, collateral contacts, travel time, paperwork, planning, knowledge building, and resource building. This position is 50% direct services, 25% documentation, 15% knowledge and skill building, 10% administrative. Job Duties and Responsibilities
: Accepts newly assigned cases in which providers within Emerald have completed diagnostic intake. Completes comprehensive targeted case management assessments and periodic reassessment of assessments on all clients entering caseload. Develops appropriate treatment plans while partnering with clients to ensure the implementation of appropriate services and build an alliance. Provide ongoing case management that is supportive and in accordance with the client’s needs, objectives, and goals identified in the treatment plan. Makes necessary revisions to the treatment plans when new issues arise to ensure the client’s needs are being addressed. Participates in planning and service coordination activities to improve and enhance service continuity and effectiveness for clients. This includes services provided by Emerald and services provided by community partners. Maintain close communication with intake, medical, and counseling staff to alert these members of staff of any changes in the client’s progress. Prepares and maintains client records, including treatment plans, case notes, and needed assessments. This includes achievement and progress documentation within each of these domains. Demonstrate ability to interview and assess clients to evaluate needs with the use of a needs assessment and develop appropriate treatment plans. Identify community resources that would be useful to clients, as well as link clients to those resources. Establish effective and positive working relationships with internal staff and community partners to ensure no barriers are created in getting a client’s needs met. Ensure all documentation complies with Kentucky Medicaid and accreditation standards. Responsibility to be aware of and follow, state reporting requirements for dependency, abuse, and neglect. Knowledge of coping skills, de-escalation techniques, and strategies to enhance client motivation. Ability to work independently and gauge priorities effectively. Participate in weekly staff meetings. Provide case management services for a maximum of 25 individuals at a time. Performs other duties as assigned by the supervisor, which are consistent with the position and in compliance with agency policies and procedures. Qualifications
: Bachelors of Science or Bachelor of Arts in Social Work, Sociology, Psychology, or other related human services fields. Must complete the targeted case management training program within six months of hire date and maintain certification requirements for TCM pursuant to Kentucky Board regulations. Excellent communication skills, a good understanding of technology, and community engagement skills are essential. This also includes the ability to coordinate meetings. Ability to work a flexible schedule that adheres to the client’s needs and support network. Access to personal, reliable transportation.
