Case Manager
The Salvation Army Great Lakes Division, Detroit, Michigan, United States, 48228
Provide case management services for the clients of The Salvation Army Harbor Light.
Responsibilities
Conducts didactics/educational lectures
Maintain case records in accordance with Harbor Light standards
Serve as Case Manager to treatment clients
Assists clinical staff with various tasks as assigned (i.e. make packets, assist clients with nonclinical issues, etc.)
Participates in the review of all clinical issues, activities and program policies and procedures
Attends all clinical staff meetings, and team conferences
Participates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skills
Qualifications
Ability to determine needs and to work with multi-problem individuals
Ability to develop rapport, trust and confidence in client relationships
Must possess good communication and teamwork skills
Strong organizational skills
TB Test and police clearance required
Authorization to work within The United States
Education/Experience
A bachelor's degree in social work or an equivalent comparable human services field is required
Experience in case management preferred
Experience with mental health and/or substance abuse populations preferred
Computer Skills
Excellent computer skills required
Experience using electronic medical records systems preferred
Certificates and Licenses
Must possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 2 weeks of hire date
Valid Michigan Chauffeur License and approved by The Salvation Army MVR
Physical Demands
Ability to lift 10 pounds
Frequent sitting for long periods of time
Capability to inspect packages, equipment, and fixtures, as well as observe people and events
The ability to hear and understand information and ideas presented through spoken words and sentences
Capability to use standard office machines like copiers and fax machines
In addition Must be highly organized, motivated, a self starter with the ability to multi-task, be creative.
