STATEMENT OF THE JOB:

Under the direction of the Clinical Director, the Senior Mental Health Clinician renders a full range of clinical, rehabilitative, recovery, case management, and support services to MI clients. The Social Worker works in collaboration with the client and staff to ensure treatment adherence by monitoring medication, reminding clients of appointments, offering health care escorts, and making necessary referrals. Review referral packages and conduct comprehensive psychosocial assessments to determine the consumer's medical, psychiatric, housing, and other social needs in the community. Conduct intake (including communicating/coordinating with government and community partners). Monitor, evaluate, and record client progress with respect to treatment goals. Modify treatment plans according to changes in client status. Establish collaborative working relationships with inpatient treatment teams and plan with them appropriate discharges for clients. Foster relationships with community providers to ensure that clients relate to appropriate services as they transition back into the community. Counsel clients in individual and group sessions to assist them in dealing with substance abuse, mental and physical illness, family issues, housing problems, and unemployment. Identify need/interest and develop and facilitate group therapy as well as adult education workshops using the rehabilitation recovery model. Escort consumers, when indicated, on appointments with community-based treatment providers. Provide ongoing psychoeducation to clients, their families, and others within their support network. Attend and participate in team meetings and supervisory sessions. Report all incidents as outlined in Agency policies and procedures manual in a timely and professional manner. Collaborate with Agency and program staff to develop and adhere to on-site emergency response preparedness and readiness protocols. Perform other related duties as assigned. Job Requirements

Master's degree in mental health counseling, psychology, or human services field. New York State Licensed Mental Health Counselor. A minimum of 3 years post-masters work experience in the mental health field. Knowledge of mental illness and serious emotional disturbances and substance abuse disorders. Knowledge of treatment, rehabilitation, and community support programs as they relate to consumers/residents, families, and staff. Knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques. Ability to work flexible hours and days - including weekends/evenings/holidays according to needs of a 24/7 program. Regularly required to talk, hear, walk, stand & sit and lift to 10 pounds. Able to operate a computer keyboard, mouse, & office equipment. Able to travel to multiple locations as needed. BENEFITS:
