Our client is a 25-bed, non-profit community hospital designated as a Critical Access Hospital. Located in Wolfeboro, NH, the hospital provides primary care, diagnostic testing and specialty services for the region. It serves a year-round population of 30,000 residents and approximately 120,000 seasonal residents and visitors from the Eastern Lakes Region and surrounding towns including Alton, Brookfield, Effingham, Freedom, Madison, Moultonborough, New Durham, Ossipee, Sanbornville, Sandwich, Tamworth, Tuftonboro, Wolfeboro and nearby communities. WHO YOU ARE: Under the general supervision of the Director of Rehabilitation and Diagnostic Services,

our Physical Therapist is responsible for administering Physical Therapy services including evaluations, establishment/modification of treatment plans, provision/supervision of treatment, and documentation for inpatients and outpatients in accordance with departmental and facility policies. The aim is to restore function, alleviate pain, and prevent disabilities by planning and administering medically prescribed physical therapy. This is a full-time position working 30 hours per week. This position is eligible for participation in medical benefits. Responsibilities

Clinical Evaluation

Validates prescription from provider is present and understood prior to evaluation and treatment; provides evaluation and treatment according to the provider7s order; calls provider if clarification is needed Performs evaluations and reevaluations according to departmental policies Evaluates effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient progress; recommends adjustments and modifications Uses objective measures on all evaluations and documented findings. Write measurable/functional goals based on objective findings Develops treatment plan with input from the patient and according to the provider7s order

Treatment and Discharge Plan

Addresses the needs of the patient including age, physical, psychological and learning needs. Encourages patient participation in plan of care and goals Uses a variety of therapeutic exercises, functional training, manual therapy techniques, assistive and adaptive devices, and physical agents and electrotherapeutic modalities Educates patient on ongoing goals as they are achieved or modified and on impending discharge Completes discharge planning by consulting with physicians, nurses, social workers and other health care workers; contributes to patient care conferences Ensures continuation of the therapeutic plan after discharge by designing home exercise programs and instructing patients, families and caregivers; recommends outpatient or home-based options as appropriate

Quality and Customer Service

Greets and welcomes all patients by name and on time Ensures documentation is complete and thorough in accordance with departmental policies and patients7 insurance requests Manages charges daily to help ensure accuracy Participates in mandatory competencies and monthly chart audits Maintains professional and technical knowledge by attending educational workshops and professional activities Supports the development of physical therapy staff through information sharing and in-service training

Other

Complies with federal, state and local legal and professional requirements; informs management of needed actions Keeps current with insurance demands, restrictions, and authorizations; educates patients about their insurance Teaches/ leads physical therapy students and educates other health care providers about Physical Therapy capabilities Directs and supervises PTAs and maintains ongoing communication Collaborates with other health professionals for patient wellbeing May travel to off-site approved locations to provide therapy services and wellness programs

Minimum Education and Work Experience

Education: Bachelor7s and/or Master7s and/or Doctorate degree in Physical Therapy from an APTA-accredited institution Experience: 3-5 years in hospital and outpatient settings Licensed in NH by the NH State Board of Allied Health CPR certified Additional Information
