We offer personalized housing, health, and human services for children and families, veterans, individuals with disabilities, and senior adults.

Founded in 1896, the organization has grown to operate 40 programs addressing our communities’ most pressing issues.

Mission: Case management collaborates with formerly homeless clients to foster self‑reliance, providing advocacy and services to help them achieve housing and independence.

Key Responsibilities

Build high‑quality relationships with participants to understand the personal experiences of those impacted by homelessness.

Administer the Youth VI‑SPDAT, the standardized housing intake.

Complete developmentally appropriate assessment goals with youth as determined by needs.

Provide assistance with obtaining mainstream resources such as SSI, SNAP, etc.

Provide training to client following goals developed in their service plan, including money management, personal hygiene skills, grocery shopping, housekeeping, meal preparation, laundry skills, and community integration.

Develop a housing plan with each youth and coordinate with appropriate programs depending on housing placement.

Provide housing search support to youth in partnership with housing staff.

Assist with move‑ins, move‑outs, and housing transitions.

Implement core principles and evidence‑based practices, including Trauma Informed Care.

Provide one‑on‑one CM activities to assist in meeting goals, including home visits, apartment checks, and supervision – 20-25 hours of face‑to‑face time each week is expected.

Document services and training provided in Service Point HMIS/LSNDC and client’s file as needed; provide accurate, complete, and timely data and reports to program management.

Collaborate and coordinate educational, vocational, employment, and career development activities.

Provide for independent living and life skills training to youth and document these events.

Engage and connect youth with appropriate behavioral health supports; communicate information necessary to the wellbeing of clients.

Ensure client confidentiality per HIPAA standards.

Complete critical incident reports as needed. Follow policy for reporting.

Identify and handle crises and contact Supervisor for needed support and directions.

Attend appropriate community meetings and all other activities identified as necessary to meet program outcomes.

Act as liaison with client’s family or next of kin, notifying family of changes or concerns about the client.

Develop and maintain positive relationships with agency staff, outside agencies, and local community.

Be on‑call for crisis situations.

Identify and report maintenance problems to the Property Manager.

Complete any other duty as required by Volunteers of America.

Transport clients to appointments or community resources.

This is a full‑time position with benefits.

Minimum Qualifications

A passion for the Level Up mission to collaborate with youth to foster self‑reliance, a belief in the capacity for a young person to change.

Bachelor’s degree in Social Work or related field.

Demonstrate honesty and authenticity.

Ability to set limits and boundaries that maintain the helping role and assist others with setting healthy boundaries.

Ability to communicate effectively with clients, co‑workers, volunteers, and supervisor.

Seek to improve knowledge, skills, and abilities in related service areas.

Must be available to work flexible hours.

Must be able to work 40 hours per week.

Preferred Qualifications

Proven ability to apply crisis intervention and de‑escalation skills.

Ability to apply theories of Motivational Interviewing, Harm Reduction, and Trauma Informed approaches.

Experience working with homeless or vulnerable populations.

Other Requirements

Must have a valid Driver’s License with good driving record and be able to operate a motor vehicle.

Liability insurance that meets minimal requirements.

Vehicle must display current safety inspection sticker.

Criminal background and driving record will be checked.

Physical Requirements Must be able to walk, sit, stand, stoop, bend, climb stairs, see, hear, and speak.
