Screen, evaluate, and re-evaluate patients Develop and revise plans of care in collaboration with supervising physician Discharge planning and ongoing care adjustments as needed Supervise Physical Therapy Assistants as applicable Maintain knowledge of Functional Pathways policies and procedures and clinical programs Qualifications
