Physical Therapist, Home Health at CenterWell Home Health. This role involves delivering personalized home-based physical therapy to help patients regain strength, mobility and independence. Base pay range
$85,400.00/yr - $117,500.00/yr Sign-on bonus:
$5,000 (associated with this role). Responsibilities
Plan and administer prescribed skilled physical therapy treatment and training for patients with injuries, illnesses, or functional disabilities to maximize physical function. Screen and test the patient’s physical strengths to assist the physician in evaluating function and establish measurable treatment objectives. Develop and implement a conditioning/rehabilitation program aligned with the physician’s Plan of Treatment and overall rehab goals; adjust treatment as needed to achieve results. Collaborate with physicians and clinical team members to obtain patient information and revise therapy plans as needed. Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides; supervise and evaluate staff performance; ensure documentation is completed accurately. Document care observations, interventions, and evaluations promptly; support interim orders from the physician and therapist with proper documentation and implementation. Communicate patient progress to the physician, Clinical Manager, staff, patient, and family; submit evaluation, treatment plans, progress reports and discharge summaries. Qualifications
Degree from an accredited Physical Therapy program (approved by the APTA). Minimum of one year of physical therapy experience preferred. Current and unrestricted Physical Therapy license. Current CPR certification. Strong organizational and communication skills. Schedule & Benefits
