The role is part of the Rushford Center, Connecticut’s leading provider of addiction and mental health services, offering a full continuum of care including acute, residential, outpatient, and community support programs.
Job Summary Work in coordination with interdisciplinary teams to organize, prepare, and deliver evidence‑based clinical services across the continuum of care. Maintain ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics.
Job Responsibilities
Conduct a comprehensive bio‑psychosocial assessment and establish a care plan.
Collaborate with the interdisciplinary team to assess appropriate level of care, develop and implement care and discharge plans, and coordinate consults and referrals to remove barriers and promote optimal clinical and financial outcomes.
Utilize specific therapeutic methods, strategies, and interventions in accordance with an individualized care plan.
Monitor progress of interventions and revise planning to meet patient and family needs.
Provide education to patient and family regarding disease prevention, impact of illness, advocacy for benefits, health maintenance, and adherence to treatment regimens.
Provide direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention and safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end‑of‑life care, referral and discharge planning.
Refer patient to appropriate levels of care and available resources that promote the individualized care plan.
Maintain timely patient‑centered documentation that reflects the patient and client systems’ pertinent information for assessment, interventions, and outcomes in accordance with regulatory licensing requirements and third‑party agencies.
Participate in administrative and development activities including team meetings, clinical supervision of staff, and staff education.
Support the development of hospital and or unit‑based programming to meet the clinical and strategic needs of the patients and the milieu.
Work as a team member to contribute to improved patient experience.
Ensure the cost‑effective utilization of resources.
Qualifications
Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or a related human service field.
Previous experience in a healthcare setting.
LMSW, LPC‑A, LMFT‑A, or CADC credentials.
Valid driver’s license.
Computer proficiency.
Ability to assess problems, determine solutions, and make decisions within a timely manner.
Utilize supervision and team approaches in the context of patient assessment and clinical interventions.
Excellent communication skills with the ability to establish relationships within and outside the entity.
Demonstrate competencies, including knowledge and behaviors, to meet age‑specific patient needs.
Focused on personal, patient/client and unit/staff safety.
Assume responsibility for self‑improvement by identifying, communicating, and seeking resources or instruction to meet own learning needs, and be self‑directed in maintaining currency of validations, certifications, licensure and training.
Recognize and address health disparities, inequalities, and injustices directed toward clients, related to access to care and provision of health services.
Develop and maintain cultural competence with an understanding of the history, traditions, values, and family systems of client groups as related to health care and decision‑making. Display sensitivity to and awareness of the diversity in cultural groups and integrate this knowledge into practice.
