Hospital at Home Teams – Victoria Hospital
The Hospital at Home (H@H) Service in Fife provides acute care for frail elderly patients at home, preventing admission or facilitating early discharge after hospital admission. This role is part of the core H@H Service and will rotate between in‑reach work at Victoria Hospital and community H@H teams. You will support discharges, carry out clinical assessments and ensure smooth transitions for patients. You will also educate colleagues about the H@H service. Responsibilities
Deliver safe, effective acute medical care mainly in the acute setting and support the wider H@H service in the community. Work autonomously in a rapidly evolving service, carrying out clinical assessments, venepuncture, ECG, IV additives and cannulation. Travel within the area to support the H@H service and respond to deteriorating patients. Rotate 7‑day shifts (08:00 – 18:30) at Victoria Hospital and community teams; attend 08:00 – 22:00 H@H main service times. Complete a minimum 3‑month period in community H@H teams before being based at the acute site. Qualifications
Degree‑level qualification in clinical decision making. Desire and ability to work toward an NMP qualification. Experience with venepuncture, ECG, IV additives and cannulation. Excellent communication, confidence, assertiveness and time‑management skills. Hospital at Home – Adamson Hospital, Cupar (East)
This role supports the H@H team at Adamson Hospital, delivering safe, effective, person‑centred care for older adults in the primary‑care setting, focusing on urgent and unscheduled care and complex management of acutely unwell, palliative or frail patients, including nursing and care‑home residents. Responsibilities
Provide community based healthcare for older adults, managing complex cases. Travel across Fife to attend to acutely unwell patients. Qualifications
Clinical assessment and Decision Making module; preferably Level 11 Non‑Medical prescribing. Trainee Nurse Practitioner applicants will be on Annex 21 until competent clinical decision making is achieved. Excellent communication and ability to work autonomously with initiative, clinical judgement and decision making. Application & Contact
