Overview
PH2S/HealthNow Clinical Research Coordinator (CRC) coordinates patients’ activities to facilitate the delivery of health care services and transition of care across the continuum in the Emergency Department’s PH2S/HealthNow Program. The CRC intervenes with populations most at risk for threats to health and well-being at the earliest opportunity and provides linkage to care to improve health outcomes. Responsibilities
Coordinate a wide range of clinical-support, recruitment of study participants and administrative functions to fully support the program. Interview patients to elicit health needs history; administer questionnaires in a courteous, professional and empathetic manner; conduct point of care testing and coordinate testing for HIV and Hepatitis C; refer to appropriate community resources and provide linkage to care; maintain patients records ensuring strict confidentiality; maintain tracking and reporting systems; collect, analyze and QA data for inclusion in progress reports. Carry out successful screening, intervention and linkage to care of patients presenting to OSU Wexner Medical Center facilities for health-related services such as addiction, pain, infectious diseases, harm reduction education; maintain communication with patients and coordinate linkage to care post-discharge; work in a variety of settings as approved by medical director, HealthNow manager, stakeholders and administrators to promote implementation, system redesign, education and quality initiatives. Implement day-to-day HealthNow activities, follow-up with patients post-discharge, and screen/enroll patients for various research studies. Receive referrals, review lab reports to determine eligibility for HealthNow program activities; complete required ROIs, provide appropriate linkage to care, and maintain ongoing communication with community partners; consent and enroll participants in eligible research studies. 70%: Clinical Role
Address patient concerns through active listening and escalate as needed to the care team. Provide linkage to care for diseases of public health importance and ensure referrals are completed and patients’ needs are addressed. Stay up-to-date on available services/programs both within and outside the OSU network to match patient needs with resources. Maintain tracking and reporting systems and complete other related duties. Use motivational interviewing (MI), open-ended questions and client-centered counseling to collect patient information and ensure necessary information is passed to the care team. This role involves multi-disciplinary teams and collaboration. 30%: Research Role
Provide assistance in support of clinical research studies; participate in recruitment of study participants; collect and provide relevant data for eligibility assessment; assist with informed consent; assist with cognitive testing; assist PI and sub-investigator with continuity of care of study patients during and between clinic visits; assist with data collection, entry, maintenance and reporting; enter protocol, medical history, and demographic information into data capture systems; enter patient clinical data into the electronic medical record; review data and progress notes with the PI and sponsor agencies; prepare protocol files for data audits and assist with development of data collection plans and forms for new protocols; prepare summaries of research results for protocol evaluations and dissemination. Minimum Qualifications
Bachelor’s degree in public health or related field (or equivalent experience). Community Health Worker (CHW) certification preferred but not required.
