Job Responsibilities
Planning, provision, and coordination of Physical Therapy to patients.
Evaluating the patient’s level of physical functioning and providing training or assistive devices to address deficits.
Working with other professional staff to establish rehabilitation goals and activities.
Evaluating PT needs, providing services per MD order, and submitting written evaluations.
Performing clinical tests, developing a Plan of Care, and setting restorative/maintenance goals.
Instructing patients, families, caregivers, and the team on assistive modalities and PT procedures.
Observing, recording, and reporting to MD and agency personnel; communicating progress.
Re‑evaluating patients as needed and completing written progress reports.
Identifying equipment issues and following up with vendors.
Submitting discharge summaries with outcomes and recommendations.
Minimum Education
Graduate from a Physical Therapy curriculum approved by the American Physical Therapy Association or the Council on Medical Education of the American Medical Association.
Minimum Experience and Skills
At least 1 year of experience as a PT in an Acute Care setting (2 years preferred); Home Care experience desirable.
Demonstrated knowledge of current PT standards, procedures, and techniques.
Outstanding assessment and technical skills.
Organized and culturally competent.
License/Registration/Certification Required
Current license as a Physical Therapist in New York State.
