Posting Summary Rutgers, The State University of New Jersey, is seeking a Community Health Worker for the Hematology/Oncology Department within New Jersey Medical School. The Community Health Worker (CHW) works closely with the EPWC Coordinator as well as our collaborators to identify persons in need of preventive services, to refer such persons to such services, and to increase knowledge of cancer, chronic disease, and COVID-19 through education, health-related programming, prevention, and screenings. The CHW also works toward increasing the health literacy of the populations of Essex and Passaic Counties, as well as other areas statewide. CHW should have deep roots or similar lived experiences to the residents of the communities they will serve. The CHW acquires and maintains knowledge of relevant preventive health resources, maintains detailed, confidential records of persons they refer to such resources, and aids in developing relationships, trust, and mutual respect and understanding between the EPWC and multiple communities in Essex and Passaic Counties, as well as in other areas of New Jersey in conjunction with our collaborators.
Among the key duties of this position are the following:
Identify, catalogue and develop relationships with community resources for cancer screening, HPV immunization, smoking cessation, COVID-19 immunization and testing, and other health related resources, as well as transportation and other necessary ancillary resources.
Through continual contact with communities, identify persons in need of cancer screening, HPV immunization, smoking cessation, and COVID-19 immunization and testing, and refer such persons to relevant screening, immunization, and/or smoking cessation resources.
Attends and supports community-based outreach, recruitment, and screening events to assist in connecting eligible persons to risk-appropriate cancer screenings, immunizations, smoking cessation and/or other health services.
In collaboration with providers of preventive services, contact and remind persons in need of follow-up services.
Assists the coordinator in the development of and the upkeep of a community resource directory, including transportation resources.
Qualifications Minimum Education and Experience:
High School Diploma required.
Certifications/Licenses:
Required Knowledge, Skills, and Abilities:
Demonstrate an ability to read and write well in English.
Demonstrate strong communication and interpersonal skills with diverse populations.
Demonstrated awareness of cultural competence and the value of such competence.
Excellent time-management and coordination skills, including setting priorities on work assignments.
Demonstrate basic computer skills and a working knowledge of Microsoft Office.
Demonstrate knowledge of and/or strong community ties to either or both of Essex and Passaic County.
Within one week of the start of employment, completion of CITI human subjects protection training and HIPAA training (both offered through Rutgers University).
Completion of Community Health Worker training that is required or recommended by the NJ Department of Health, Screen NJ, or other grantors funding this position, as soon as such training is made available, or if immediately available upon hiring, within two weeks of the start of employment.
Possession of a valid current NJ driver's license.
Have full-time access to a reliable vehicle to facilitate the daily duties at multiple locations (mileage and tolls reimbursed at standard University / federal rates).
Preferred Qualifications:
Bilingual or Multilingual (especially Spanish, Haitian Creole, Portuguese, and/or Arabic).
Previous educational or work experience within any of the following fields: community health, health education, health promotion, medicine, nursing, psychology, public health, or a related field.
Knowledge of health issues: colorectal cancer screening, HPV immunization, lung cancer screening, smoking cessation, COVID-19, and barriers to receiving and following up with healthcare and medical treatments.
