Functional Pathways is currently hiring at our newest location, Covenant Living of the Great Lakes in Grand Rapids, MI, for a PRN Physical Therapist. Weekend availability is needed. Job Summary
The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician. Responsibilities
Provide physical therapy services per plan of care and physician supervision. Screen, evaluate, re-evaluate, and modify care plans as needed. Discharge planning and revisions to the established plan of care. Qualifications
Bachelor's, Master’s, or Doctorate in physical therapy from an accredited program. Certification and licensure requirements as established by state and national board associations. Active state license required. Excellent organizational and time management skills. Excellent written and verbal communication skills. Working knowledge of Functional Pathways policies and procedures, and clinical programs with application to therapy disciplines. Ability to supervise Physical Therapy Assistants. Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
