Physical Therapist – BoldAge PACE Program The Physical Therapist at BoldAge PACE provides comprehensive physical therapy services to elderly participants, focusing on assessment, treatment, and education to improve independence and quality of life. The role involves collaboration with an interdisciplinary team, performing home safety evaluations, recommending adaptive equipment, and documenting progress. Licensure, clinical experience with elderly or frail populations, and a commitment to ongoing professional development are required.

Job Summary Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include assessment, planning, providing treatment, and teaching participants, caregivers, or other appropriate representatives/family to maintain community support.

Essential Duties and Responsibilities

Provides high‑quality care and serves as a member of the PACE interdisciplinary team (IDT).

Performs in‑person assessments for participants at initial enrollment and as needed.

Assesses the need for adaptive durable medical equipment, specialized shoes, and prosthesis; evaluates participants’ homes for safety and mobility; recommends home modifications and equipment.

Plans physical therapy treatment programs according to recognized standards and policies.

Administers treatment programs consistent with achieving identified goals and standards; conferences with IDT on care issues.

Documents evaluation, assessment, plan, and treatment rendered accurately and in a timely manner.

Measures effectiveness of treatment, documents and revises plan accordingly.

Educates and instructs participants/family in procedures, home programs, and documents training.

Trains PACE staff in body mechanics, equipment use, transfers, gait patterns, and methods to maximize safety and functional abilities in daily living activities.

Provides care and services in the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care.

Assists participants to function at the most independent level possible.

Develops and maintains positive relationships and communication with co‑workers, participants and families, and community members.

Assists IDT members in understanding the rehabilitation needs of participants and families.

Collaborates with IDT to develop a comprehensive care plan for each participant.

Maintains regular attendance at and participates in daily IDT meetings, communicating participant changes and collaborating in care planning decisions for 24‑hour delivery.

Performs duties of other IDT members as needed based on licensing, competencies, and experience.

Actively participates in quality improvement projects and meetings.

Fulfills the role of other IDT member(s) as required by licensure and competency.

Attends and actively participates in organizational meetings related to participant care or daily operations, in‑services and community agency meetings.

Completes all documentation of clinical service in participants’ medical records per PACE requirements.

Follows all policies, procedures, and OSHA safety guidelines.

Protects privacy and maintains confidentiality of all company procedures, results, and information about employees, participants, and families.

Practices standard precautions and maintains a safe working environment.

Participates in continuing education classes and required staff training meetings.

Maintains professional affiliations, required certifications, and continuing education requirements.

Other duties as assigned.

Experience, Education, and Certifications

Advanced degree in Physical Therapy from an accredited school.

Current license to practice Physical Therapy in the state.

Current driver’s license and proof of auto insurance.

Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home, or community‑based setting (preferable).

One year of experience working with a frail or elderly population (preferred; training provided if not present).

Work Schedule Monday‑Friday 8:00 am‑4:30 pm. Full‑time.
