Position Summary Under general supervision, will support population health and quality initiatives with a focus on preventative health care measures aimed at improving health outcomes through telephonic and written communication. Primary duties of this position include performing telephonic patient outreach to schedule appointments, obtaining health records from external providers, and sending reminders to patients to complete chronic care gaps as identified by the Care Manager. This position requires an administrative focus with the vast majority of patient interaction occurring via telephonic outreach.

Primary Duties & Responsibilities

Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient‑centric, promoting quality and efficiency, and the delivery of healthcare for medical home patients.

Assesses, plans, implements, coordinates, monitors, documents, and evaluates options and services to optimize the member’s health status.

Develops systems of care that monitor member’s progress and promotes early intervention in acute care situations.

Establishes and promotes a collaborative relationship with health care providers, payers, and other members of the health care team to optimize interventions.

Manages utilization and practice metrics to refine the delivery of care model to maximize clinical, quality, and fiscal outcomes.

Completes necessary paperwork and documentation in a timely manner.

If order entry in the electronic medical record is part of the job function, completes requisitions/orders per WashU guidelines.

Additional responsibilities include providing proactive care outreach to patients overdue for preventative health services to support value‑based care initiatives. NCM will collaborate with providers, clinical staff, social work, case management, and community resources to assist patients with barriers to care. NCM will be responsible for developing and implementing patient‑centered strategies to improve management of chronic conditions, improve clinical outcomes, and enhance patient satisfaction.

B.S.N. - Bachelor Of Science In Nursing

Certifications:

Basic Life Support - American Heart Association, Basic Life Support - American Red Cross, Registered Nurse - Illinois Department of Financial and Professional Regulation, Registered Nurse - Missouri Division of Professional Registration

More About This Job Required Qualifications:

Registered Nurse in the state of Missouri or Illinois.

Salary Range $60,800.00 - $105,700.00 / Annually
