Coordinate patient care by integrating assessment, planning, implementation, and evaluation in collaboration with patients, families, physicians, and support services Collect and prioritize health data for physical and psychosocial assessments, involving patients and relevant parties Identify nursing diagnoses and expected outcomes based on assessments, prioritizing by patient condition Develop and document individualized plans of care in collaboration with interdisciplinary teams Implement interventions aligned with the care plan, ensuring timely treatments, medications, and education Delegate tasks appropriately while maintaining oversight and accountability Reassess patient condition regularly, updating care plans and evaluating treatment effectiveness Maintain documentation for assessments, interventions, and education provided Contribute to discharge planning and coordinate with support services Perform other duties as assigned What Sets BRG Apart
