Job Overview
As a Licensed Practical Nurse (LPN) at Paramount Community Living & Rehab, you will play a crucial role in delivering high-quality, compassionate care to our residents. Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well-being of our residents. Responsibilities
Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician. Monitor residents’ vital signs, document progress, and report significant changes to the Charge Nurse or Director of Nursing. Assist in developing and implementing individualized care plans tailored to each resident’s needs. Maintain clear and accurate documentation of patient care and communicate effectively with residents, families, and the healthcare team. Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols. Provide emotional support and education to residents and their families, helping them understand care plans and health conditions. Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated and comprehensive care. Qualifications
Valid LPN license in Kansas. Accredited LPN program graduation. Previous experience in a skilled nursing facility or similar healthcare setting preferred. CPR/BLS certification required; additional certifications may be required based on state regulations. Excellent verbal and written communication skills; ability to work collaboratively with a diverse team of healthcare professionals. Physical ability to lift and move patients, perform physical assessments, and stand for extended periods. Benefits
