The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.
The responsibilities of the Nurse Practitioner include, but are not limited to:
Responsibilities
Participates in the health assessment and examination of BCSC PACE applicants and participants.
Serves as a member of the PACE Interdisciplinary Team
Evaluates the needs and participates in the development of a comprehensive program and nursing treatment for each participant in the BCSC PACE Clinic.
Manages minor illnesses, injuries, and the stable, chronic illnesses of participants, according to standard procedures.
Evaluates the acute illnesses of the participants.
Coordinates appropriate referrals to health care providers in consultation with BCSC PACE physician.
Makes nursing home/or home visits when appropriate and as directed by the BCSC PACE physicians.
Provides clinical assistance to BCSC PACE's staff registered nurses.
Participates in all programs of health restoration, prevention, promotion and maintenance of health.
Attends and participates in staff meetings.
Duties may include participation in the Inter-Disciplinary Team (IDT) meetings as a proxy and fulfill responsibilities as assigned.
Fulfills all administrative requirements, including statistical records, chart entries, etc.
Meets with department directors on a regularly scheduled basis, and conduct/participate in in-service classes and supervisory level training programs.
Attends and participates in workshops, seminars, etc., to keep abreast of current changes in the long-term care field, as well as to maintain a professional status.
Attends and participates in annual in-service training programs for hazard communication, TB management, and blood borne pathogens standard.
Actively participants in the Quality Assessment and Performance Improvement (QAPI) process. Consistently meets or exceeds BCSC quality assessment and performance improvement targets.
Continually seeks better ways for delivering services and communicating with participants while demonstrating proficiency in delivering age-specific care.
Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality and independence.
Understands the importance of community involvement and participates as appropriate in activities that link BCSC to its communities.
Consistently meets or exceeds BCSC targets for productivity and customer service targets.
Ensure Nursing personnel, on your tour of duty, participate in all fire safety and disaster preparedness drills in a safe and professional manner.
Experience and Skills Certificates & Licenses:
Licensed RN & NP, State of California; meets standards of Board of Registered Nursing and the appropriate Business and Professional Code, State of California; successful completion of a program of study, which conforms to Board of Registered Nursing standards; verification of completion of a CPR (BLS) and first aid training. TB screening and successful completion of a health examination by a health care provider is required for employment.
Experience:
One year of nursing experience at the RN level in a busy clinic and some supervisory experience preferred. At least one year of experience working with a frail elderly population.
Skills & Aptitudes:
Effective communication skills, both written and verbal, are required. Accuracy, attention to detail, and consistent follow-through are required. Willingness to work within the framework of a multidisciplinary team in the BCSC model of care. Bilingual Spanish or Korean preferred.
