for patients recently discharged from hospital, promoting safe and effective transitions back into the community.
Develop and deliver tailored rehabilitation plans
that address individual patient needs and optimise recovery outcomes.
Collaborate closely with multidisciplinary colleagues , ensuring integrated and person-centred care.
Promote independence and mobility , empowering patients to achieve their functional goals.
Maintain accurate and detailed documentation
in line with NHS and professional standards.
This role requires registration with the
Health and Care Professions Council (HCPC)
and a valid
UK driving licence
to undertake community visits efficiently.
