Base pay range $44,000.00/yr - $53,000.00/yr

SAFY is seeking a full‑time Case Manager I for our Lexington division in Kentucky. As a Case Manager, you will be responsible for the case management functions of a caseload of foster youth and homes. You will work closely with youth, family, foster parents and community partners, equipping the youth and foster parent with the support and resources needed. This position is hybrid with travel expectations (50‑70% of the work week involves traveling in and around Lexington).

What You’ll Do

Travel daily to provide in‑home, community‑based services to youth in caseload.

Complete referral reviews and intake assignments, and communicate service availability to all assigned clients.

Provide instruction, intervention, and assistance as needed through independent judgment and as part of the treatment team to help the youth live independently.

Build and maintain relationships with youth, state workers, and community supports.

Coordinate and monitor service provisions for youth in the community and with employers, school officials, courts and other appropriate parties involved with the client.

Develop and ensure completion of appropriate assessments including Individualized Treatment Plans, Psychosocial Diagnostic Assessments (PSDs), and Mental Health Services Plans according to established time frames involving all appropriate parties as required by policy and regulations of the state and/or contract.

Timely document outcomes of home visits, support sessions, safety assessments, interaction with parties involved in the youth’s care and incident reports as required by policy, regulations, and contract requirements.

Ensure foster parents, youth and case manager needs are met during periods of staff transition.

Attend rotating on‑call schedule.

Participate in performance quality improvement processes and execute any assigned tasks.

Successfully complete all required training pertaining to the job.

Establish and maintain a positive working relationship, sensitive and responsive to the cultural differences with all levels of SAFY staff, clients, families and vendors.

Work nontraditional hours daily/weekly to fulfill position duties; may include nights and/or weekends.

Maintain confidential and sensitive information.

Other duties or special projects as assigned.

Qualifications

Bachelor’s degree in social work, counseling, psychology, sociology, child & family studies, or other approved related field of human services.

Two years of relevant experience is required with above listed degrees; experience in child welfare preferred, or relevant experience working with children in general. Five years of relevant experience required with related field of human service degrees.

Must have a valid driver’s license, acceptable driving record and auto insurance coverage to meet travel requirements; may require transporting youth if needed.

Operate a computer with Microsoft Office, other software, agency databases and online systems.

Demonstrated skill of writing treatment plans, case notes, etc.
