Peer Specialist, Safe Options Support (SOS) Join ACMH, INC. as a Peer Specialist, Safe Options Support (SOS) to engage and advocate for homeless and precariously housed populations in New York City.

Position Overview

Daily outreach on streets and transit, coordinating participants’ housing needs, attending hospital discharge planning, collecting data, and supporting participants through move‑in and community reengagement.

Job Responsibilities

Persistent strength‑based outreach at known “hang‑outs” and transit hubs.

Assess health and social needs using SOS conversational, observational, and formal risk assessment tools.

Collaborate with the centralized SOR Hub to identify available housing, completing HRA 2010e, applying for housing, and assisting participants with move‑in and neighborhood orientation.

Participate in hospital discharge planning meetings to link patients with community resources.

Collect and report data, working with the team leader, data analyst, and other SOS teams to inform future care delivery.

Work with housing providers to resolve clinical and housing issues and retain supportive housing.

Build relationships with community providers to connect participants with services during transition back into the community.

Provide appointment navigation, accompaniment, reengagement, and barrier removal.

Conduct comprehensive psychosocial assessments, gather historical and collateral information from multiple sources.

Monitor, evaluate, and document participant progress toward care plan goals.

Attend team meetings and supervisory sessions.

Perform other related duties as assigned.

Essential Knowledge, Skills & Abilities

Experience with homeless populations preferred but not required.

Knowledge of NYC shelter systems and MTA transit a plus.

Knowledge of counseling principles for mental illness and substance use disorders.

Knowledge of treatment, rehabilitation, and community support programs that serve recipients, families, and staff.

Ability to develop, evaluate, and implement treatment interventions to meet individual needs.

Ability to prepare accurate and timely reports.

Computer proficiency and good documentation skills.

Minimum Qualifications

High School Diploma or higher. New York State Certified Peer Specialist preferred or required. Completion of a peer advocacy internship or comparable training. Experience in providing advocacy services to mentally ill and/or homeless individuals.

Salary & Benefits

$54,952 plus generous benefits.
