Shift Details: Schedule template base on every 6 weeks. Week 1: Monday & Tuesday (1300-2130), Wed (1200-2030), Fri (1300-2130). Week 2: Mon (1300-2130), Tue (1200-2030), Wed (1200-2030), Fri, Sat & Sun (1300-2130). Week 3: Tue (1300-2130) & Wed (1200-2030). Week 4: Mon & Tue (1300-2130), Wed (1200-2030), Fri (1300-2130). Week 5: Tue (1300-2130) & Wed (1200-2030), Sat & Sun (1300-2130). Week 6: Mon & Tue (1300-2130), Wed (1200-2030), Fri (1300-2130). Holidays as assigned. Schedule template includes every 3rd weekend rotation.
Job Summary
This position is responsible for facilitating patient transfers to Froedtert Hospital, Froedtert Menomonee Falls Hospital, Froedtert West Bend Hospital, and Froedtert Community Hospitals. The individual will attend emergency and non-emergency transfer requests and coordinate all transfer requests promptly and courteously. Collect pertinent clinical information from sending provider, including diagnosis/es, diagnostic test results, current treatment plan, past medical/surgical history, and the reason for the transfer. With the accepting physician and sending provider, determine appropriate level of care/location, transport mode (air/ground), and unit type. Maintain in-depth knowledge of EMTALA regulations for ED transfers and act as a resource to key stakeholders. Prioritize transfers based on patient acuity and clinical information, following established hospital workflows and protocols. Participate in difficult or critical situations and employ problem-solving and decision-making skills in a fast-paced environment. Document all transfer discussions in the EPIC system as part of the transfer documentation. Collaborate with physicians, nursing, admitting, and hospital patient flow to ensure timely transfers meeting hospital standards. Work with hospital patient flow to strategize patient placement and optimize occupancy. Communicate effectively with other team members to accomplish workload. Facilitate consult requests from outside facility providers with Froedtert & MCW physicians, document on the provider’s behalf, and attend to transfer needs resulting from consultations. When working shifts between 0000-0800, cross-train to assist with patient phone triage as needed.
Experience Description
A minimum of 4 years of nursing experience, with at least 3 years in acute care nursing. Previous critical care or ED experience as an RN is preferred.
Education Description
Bachelor of Science in Nursing is required. In lieu of a Bachelor's degree, either an additional 3 years of experience or active enrollment in a Bachelor's program with a defined end date is required.
License Description
Requires current state of Wisconsin Registered Nurse License or a Multi-State Nursing License from a participating state in the NLC (Nurse Licensure Compact).
