Purpose Statement/Position Summary The Nurse Care Manager performs complex discharge planning for patients of different ages and developmental levels with serious illnesses. Focuses on patients with complexities related to finance and/or resource utilization. Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team.
Minimum Qualifications/Work Experience 3+ years nursing experience with advanced level pediatric clinical experience and utilization management and/or discharge planning experience. Thorough knowledge of managed care, utilization management, discharge planning, and continuous quality improvement concepts. Computer literacy. Strong interpersonal and negotiation skills. Fluency in both English and Spanish desired.
Education/Licensure/Certifications Bachelor’s Degree in Nursing required. Master’s degree in Nursing, Health Care Administration, or related field preferred. Current Registered Nurse licensure in the State of California and AHA CPR/BLS certificate required.
Pay Scale Information $116,189.00–$190,882.00
