The Senior Registered Nurse (RN) with a bachelor’s degree (BSN) independently observes, assesses, plans, intervenes and evaluates outcomes and documents all aspects of patient care. Coordinates scheduling within assigned areas and provides primary management for the service area during the assigned shift, supervising and evaluating other nursing staff. Collaborates with Nurse Coordinator, other charge nurses, Psychologist, Physicians and Mental Health Supervisor to ensure 24-hour continuity of care. May assume duties of the Nurse Coordinator/Nurse Manager during absence. Flexible in work schedule. Provides direct nursing care to patients as needed. Responsibilities

Observe, assess, plan, intervene and evaluate patient care; document in the patient record to determine priorities and plan therapeutic nursing care. Evaluate patient data and medical history to assist providers with treatment plans; interpret and evaluate patient response to plans of care. Provide direction and coordination of staffing and patient care activities on assigned units; coordinate and administer effective utilization of nursing service personnel. Facilitate admissions and/or transfers; communicate with admissions staff and unit staff to ensure appropriate status and triage patients as needed. Work with providers and other health care professionals to establish and implement courses of treatment; follow provider orders to deliver and document individualized plans of care. Carry out medical staff practitioners’ orders and implement nursing interventions per standards and policies; communicate diagnosis and treatment plans to patients and families; assist in abuse and neglect investigations. Implement special precautions and restraints as ordered; maintain patient safety and uphold patient rights. Attend mandatory training; participate in ongoing in-service and education programs; monitor treatment, intervention and protection strategies (TIPS) during restraints as required. Communicate openly with leadership to disseminate current information to staff on assigned units. Perform other duties as assigned, including disaster response, COOP activation, and potential alternate shift patterns or locations. Knowledge, Skills And Abilities

Knowledge of state and federal laws for mental health, Texas Nurse Practice Act, and Board of Nursing rules. Knowledge of emergency procedures, CPR and patient safety standards. Skill in supervising employees, nursing care, and use of nursing tools and equipment; ability to lead change and analyze problems. Proficiency with computer software and electronic health record systems; time management and prioritization; effective communication and collaboration abilities. Ability to train staff, develop policies/procedures, interpret regulations, and evaluate services for improvement. Ability to organize, coordinate and evaluate nursing activities; provide guidance to others; maintain continuing education hours. Licensure, Registrations Or Certifications

Licensed to practice as a Registered Nurse (RN) in the State of Texas (or state with reciprocity through NLC) prior to hire. If transporting patients, must have a valid Texas driver license or obtain it within 90 days; out-of-state drivers must provide a certified driving record per HHSC policy. Qualifications

Graduation from an accredited nursing program with BSN equivalent preferred; at least three (3) years of RN experience in a recognized health care facility. Other Information

Applicants may be required to pass fingerprint background checks, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, Employee Misconduct, LEIE). All applicants must be at least 18 years old; some locations may require 21+. Flexibility in work hours; overtime and extended hours may be required. Immunization policy compliance is required; vaccination may be required or protective measures may be needed. HHSC uses E-Verify; bring I-9 documentation on first day.
