Position overview: This position provides clinical coordination of health services for an assigned caseload to ensure the best possible standard of care. Responsible for promptly reporting to the supervisor any circumstances that negatively affect the health, safety, or wellbeing of residents, staff, the agency, or its property and equipment. Pay rate: $27 per hour Key responsibilities: Provides clinical training, supervision, and support as needed to fellow Licensed Practical Nurses (LPNs), Nursing Assistants, and programmatic staff. Coordinate and follow up on all major resident health appointments. Observe and chart residents' progress, behavior, and unusual incidents on a timely, regular, and consistent basis. Report unusual developments to immediate supervisor, Residential Manager/Q, Residential Coordinator, and follow Major Unusual Incident reporting protocol. Communicate and educate Residential Manager/Q, Residential Coordinator, and direct care staff as information is gathered or as health needs warrant. Assess and write summaries and reports as required on a timely, regular, and consistent basis. Follow through on lab results, dietary, therapy, pharmacy, and IHP recommendations. Attend annual IHP, special meetings, and pertinent Quarterly IHPs. Monitor trained, unlicensed direct care staff; ensure MAR timely documentation/accuracy, and check medications to ensure the safety of the delegated nursing program. Requirements: Licensed Practical Nurse with current state of Ohio licensure. One year of experience as an LPN in an acute and/or long-term care setting; prefer minimum one year experience in residential programming for individuals with developmental disabilities; OR any equivalent combination of experience and/or education from which comparable knowledge, skills, and abilities have been achieved. 18 years of age or older. Must have an OH driver's license. High school diploma.
