Clinical Coordinator
to support the delivery of care in our
OMH Youth ACT (Assertive Community Treatment) program . The Clinical Coordinator ensures that treatment plans are carried out effectively, engagement goals are met, and families receive the support and resources they need. This role serves as a vital liaison among families, clinicians, and community partners.
Responsibilities
Coordinate services for Youth ACT participants in alignment with individualized treatment plans.
Monitor each individual’s status from referral through evaluation, treatment planning, and ongoing engagement.
Ensure all required monthly contacts are met while addressing the unique needs of individuals and families.
Serve as a liaison to families, providing ongoing support and communication.
Connect families to community-based case management and other supportive services.
Act as a point of contact for external case managers, caregivers, and referral sources to ensure continuity of care.
Coordinate release and requests for information in compliance with policies and regulations.
Conduct periodic chart audits to support quality assurance standards.
Maintain cooperative relationships with community providers and CSPOA to integrate services and avoid duplication.
Demonstrate proficiency with computer systems and documentation.
Perform other duties as assigned.
Qualifications
Bachelor’s degree in a Human Services discipline
preferred .
Strong organizational, detail-oriented, and time management skills.
Excellent oral and written communication abilities.
Ability to build positive relationships with youth, families, and community partners.
Bilingual Spanish strongly preferred.
Salary:
$67,000 - $74,000; Grant Funded Program
